In a movie set in Northeast India, a priest and his daughter are traveling to a temple during a festival. Unfortunately, the daughter gets hurt and her blood releases Baak, a demon who can change shape. Baak kills her and takes her place. To protect his wife, the priest traps the demon inside a vessel, making it pretend to be his daughter forever. Meanwhile, in a nearby village, a family faces tragedy when the father dies and the mother commits suicide, leaving their kids devastated. The mother’s brother and a doctor try to help, but strange and scary things start happening around them.
Saravanan thinks his sister Selvi’s death wasn’t a suicide and goes to the police, but the inspector dismisses him rudely. Ravi, a local leader’s son, shows Saravanan a video of Selvi’s husband being followed by a sage before he died. While investigating, a spirit attacks their jeep and abducts Ravi. Saravanan finds the jeep wrecked against a tree with Ravi dead on top the next morning. Using the video as evidence, Saravanan convinces the police of his innocence and the sage’s involvement. The police then help him find the sage’s hideout, where they discover photos of all recent victims, leading to the sage’s arrest order.
Saravanan discovers Shakthi, the inspector, and the late Ravi all share the same birthdate and a red mole, marking them for a sinister fate. He learns from a sage that Baak, an evil spirit previously weakened by a priest, seeks to sacrifice these individuals to regain full power. Baak, having killed and impersonated Selvi’s husband, is thwarted by Selvi’s spirit, who sacrificed herself to protect her children. As the inspector dies tragically, Shakthi awakens from her coma under her mother’s watchful spirit. Saravanan is warned to protect Shakthi at all costs, to prevent Baak from achieving immortality.
Saravanan admits to his aunt that he regrets not supporting her earlier. Meanwhile, Baak attacks the police station, causing injuries, but is temporarily repelled by the sage. In another incident, Maaya is stopped by Selvi’s spirit while trying to attend Shakthi’s birthday party. Saravanan notices a suspicious blood stain on Maaya’s lab coat, which leads to the discovery that Baak has been impersonating the sage. They find the real sage’s body in the president’s car trunk. The president admits the sage was trying to destroy Selvi’s spirit. Despite their efforts, Selvi’s spirit vanishes, pleading for Shakthi’s safety. In a final showdown at the temple festival, Saravanan defeats Baak with the help of a divine spirit.
After the unrest is over, Saravanan tells Maaya that the palace will be turned into a hospital so she can carry on helping the community. The kids demonstrate that their mother is still keeping an eye on them by playing throwball outside with her ghost.
